Four buckets

Verified fact means the file has acceptable support. Borrower claim means someone said it but proof is not there. Risky assumption means the team is acting like something is true. Missing proof means the file may work, but the document is not in hand.

Second-round condition trap

Weak uploads create second-round conditions. If a statement cannot clear the question in the format the underwriter needs, the file is not actually closer to done.

The clean next ask

Ask for the document that converts the riskiest assumption into a verified fact or confirms it is time to pivot.
